Extracurricular Classes Permission Note for OSHC

If your child attends an after-school club or class and will be dropped off to Aftercare afterwards, we require your permission to authorise the club teacher to sign your child into the service. Please download the form below, complete it, and attach it or email it to assco-oosh@bigpond.com.
