If your child arrives at the service with an injury that occurred prior to attending our service, we ask that you complete an Injury on Arrival form. This ensures we are aware of the injury and can continue to support your child’s safety and wellbeing while in our care. Please download the form below, complete it, and return it to a staff member upon arrival.
